HPC offers coordinated services and ongoing communication among a patient’s assigned personal HPC Care Team as well as collaboration and coordination of care with a patient’s medical team via HPC’s Web Portal. HPC Web Portal

The HPC Web Portal was designed to facilitate safe and easy communication between the patient and their care team. This allows both the patient and their care teams convenient 24 hour access to treatment records in real time. Using a secure username and password, patients can view personal health information including but not limited to:

  • Schedule or view appointments
  • Request refills for medication or supplies
  • View your latest medical correspondence
  • Send a secure message to your HPC nurse, pharmacist, and account representative
  • Fill out update your medical records
  • Review account statements
  • Easily find contact information for your prescribing office and care team

HPC’s patient portal provides extra care and support in between physician visits. It also gives access to your individualized therapy plan on any device where an internet connection is available. View our Tutorial Video for help on setting up and optimizing your Patient Portal account.

HPC is committed to providing care for the whole patient – not just treating the disease. Our comprehensive system of care includes:

Clinical Management

The HPC clinical staff assesses the patient’s knowledge of the disease, complications, symptoms, and medications and provides ongoing education and training based on the patient’s need. An individual Plan of Care is developed and is updated every three months and as needed – and includes interventions and measurable goals with regard to need for services, treatment, or care. The HPC pharmacist assists with medication recommendations to improve therapeutic outcomes and medication adherence.

Medication Management

The HPC Care Team monitors and evaluates patients for taking the correct medication for their specific disease management as prescribed by their physician. Our goal is to reduce the overall cost of treatment by reducing risk of under- and/or over-utilization of medications, identifying potential problems, or preventing symptoms from progressing that would require emergent care or even hospitalization as a result of poor medication adherence.

Community and Social Services

The HPC non-clinical staff of consultants and patient advocates help coordinate community resources on behalf of the patient, their families, and caregivers. Our social services professionals assist with psychosocial and other facets related to the patient’s specific medical condition.

Financial Management

HPC automatically submits claims to the patient’s health insurance provider based on the date a prescription is filled. If a claim is rejected, the HPC Care Team notifies the patient to begin working together to resolve the issue. If unable to resolve the issue, the team provides the patient the information needed on how to file a claim themselves. HPC has a co-pay assistance referral program to ensure continuity of service, preventing avoidable emergency-room visits or hospitalizations due to an interruption in drug therapy.

HPC Patient Assistance Program

HPC will assist our patients in the enrollment/qualifying process of the various patient assistance programs. These services help patients find the financial resources needed to live with chronic illnesses or conditions and a way to satisfy the expensive co-payment/deductibles.

Some of the programs include the Patient Access Network (PAN) Foundation and Patient Services Inc. (PSI) which are both nationally known, non-profit charitable organizations that provides a “safety net” for patients to alleviate some of the financial burden due from expensive premiums, high deductibles, and co-payments. Other non-profit organizations programs are utilized as well as the man factor co-pay assistance programs.
